HIPPA & Privacy Practices
I prioritize the privacy and confidentiality of your family's health information. My practices at MAPS are aligned with the Health Insurance Portability and Accountability Act (HIPAA) and relevant Washington State laws to ensure the protection of your Protected Health Information (PHI). This includes any information that identifies you and relates to your past, present, or future physical or mental health, and any related health care services. I maintain rigorous protocols to safeguard your PHI, only disclosing it with your explicit authorization or as required by law. My commitment to privacy is unwavering, and I provide access to your records upon request, allowing you to review and correct them as necessary. If you have any concerns about your privacy rights, please do not hesitate to contact me.
I want to assure you that your family's trust and confidentiality are of utmost importance at MAPS. I will use your PHI strictly for evaluation, treatment, payment, and health care operations, always adhering to the minimum necessary principle. In specific situations, such as court orders or instances of abuse or neglect, I may be legally required to disclose information without your authorization, but I do so with the greatest care and legal compliance. My privacy practices are transparent and accessible, and I encourage you to reach out with any questions or requests regarding your PHI. Cancellations and No-show Policies
I understand that emergencies happen. If for some reason, you are unable to make it to the scheduled session, please try to cancel or reschedule at least 24-hours in advance. Cancellations and no-shows will be subjected to charge a full rate if you do not cancel or reschedule at least 24-hours in advance.
Cancellation or reschedule can be made directly by calling or texting at (425) 223-3589 (text is preferred) or emailing drma@mosaicanchorpsychologicalservices.com.
Friendly reminder: It is okay if you leave me a voicemail if you can’t get a hold of me because I might be in a session with other client when you called.
