Rates & Insurance

I am in-network with Premera Blue Cross. If you have Premera Blue Cross, I will bill the insurance on your behalf and charge any copays or deductibles after I receive the claims.

Common billing codes for my services include:

  • 90791: Diagnostic evaluation/intake assessment

  • 96130/96131/96136/96137: Testing and related tasks

  • 90837: Individual therapy, 60 minute 

  • 90846: Family session without child/teen present, 50 minutes

  • 90847: Family session with child/teen present, 50 minutes

  • 90853: Group therapy

I can provide superbills for out-of-network clients to submit directly to your insurance company for potential reimbursement. If you are interested in using your out-of-network benefits, I strongly recommend contacting your insurance provider to determine what services may be covered. If you are not using insurance, you still have the right to receive a Good Faith Estimate explaining the expected costs of your mental health care.

Out-of-network

Many families also use their FSA or HSA accounts to pay for medically-necessary services at MAPS (e.g., comprehensive evaluations, and individual and group therapy; note that standalone IQ testing or kindergarten early entrance evaluation for school admissions is not typically reimbursed by insurance or by HSA/FSA accounts due to it being educational in nature rather than medically-necessary).

Under the federal No Surprises Act, which took effect on January 1, 2022, you are entitled to better understand your healthcare costs in advance and to minimize unexpected medical bills. The Good Faith Estimate provision of this law ensures that you receive an estimate of the charges for healthcare services before your appointment. As an out-of-network provider besides Premera Blue Cross, I will provide you with a Good Faith Estimate form prior to starting therapy. This estimate may change based on your therapeutic needs and session adjustments. I deeply value transparency and your financial security, which is why my service fees are listed on my website. I will always disclose relevant fee information before your initial session and will not adjust your fees without your verbal and written consent.
